es.%20We%20conducted%20a%20synthesis%20of%20qualitative%20findings%20regarding%20care%20for%20chronic%20conditions%20at%20primary%20care%20facilities%20in%20LMICs.%20The%20themes%20identified%20were%20used%20to%20adapt%20the%20existing%20chronic%20care%20model%20%28CCM%29%20for%20application%20in%20an%20LMIC%20using%20the%20%5Cu2018best%20fit%5Cu2019%20framework%20synthesis%20methodology.%20Primary%20qualitative%20research%20studies%20were%20systematically%20searched%20and%20coded%20using%20themes%20of%20the%20CCM.%20The%20results%20that%20could%20not%20be%20coded%20were%20thematically%20analysed%20to%20generate%20themes%20to%20enrich%20the%20model.%20Search%20strategy%20keywords%20were%3A%20primary%20health%20care%2C%20diabetes%20mellitus%20type%202%2C%20hypertension%2C%20chronic%20disease%2C%20developing%20countries%2C%20low%2C%20middle-income%20countries%20and%20LMIC%20country%20names%20as%20classified%20by%20the%20World%20Bank.%20The%20search%20yielded%20404%20articles%2C%20338%20were%20excluded%20after%20reviewing%20abstracts.%20Further%2C%2042%20articles%20were%20excluded%20based%20on%20criteria.%20Twenty-four%20studies%20were%20included%20for%20analysis.%20All%20themes%20of%20the%20CCM%2C%20identified%20a%20priori%2C%20were%20represented%20in%20primary%20studies.%20Four%20additional%20themes%20for%20the%20model%20were%20identified%3A%20a%20focus%20on%20the%20quality%20of%20communication%20between%20health%20professionals%20and%20patients%2C%20availability%20of%20essential%20medicines%2C%20diagnostics%20and%20trained%20personnel%20at%20decentralised%20levels%20of%20healthcare%2C%20and%20mechanisms%20for%20coordination%20between%20healthcare%20providers.%20We%20recommend%20including%20these%20in%20the%20CCM%20to%20make%20it%20relevant%20for%20application%20in%20an%20LMIC.%22%2C%22date%22%3A%2212%5C%2F2018%22%2C%22language%22%3A%22en%22%2C%22DOI%22%3A%2210.1136%5C%2Fbmjgh-2018-001077%22%2C%22ISSN%22%3A%222059-7908%22%2C%22url%22%3A%22http%3A%5C%2F%5C%2Fgh.bmj.com%5C%2Flookup%5C%2Fdoi%5C%2F10.1136%5C%2Fbmjgh-2018-001077%22%2C%22collections%22%3A%5B%5D%2C%22dateModified%22%3A%222022-02-03T15%3A09%3A08Z%22%7D%7D%5D%7D
